- If your child has prescription meds, a chaperone must administer the medication for the duration of the trip. Please complete form “Authorization to Give Medication (English)”
- If your child has to carry their own prescription medication (cases may be asthma, epinephrine auto injector or diabetic meds), Please complete form “Authorization to Carry Prescription Medication (English)”
- If your child is going to carry Over the Counter medication, please complete form “Authorization to Carry Over The Counter Medication (English)”
Airport Drop Off/Pick Up Authorization Form - if an adult other than the parent/legal guardian of your student is dropping off or picking up your student at the Airport.
